Start with accurate, timely data
You can't close what you can't see. Gap lists are only as good as the data behind them. Reconciling claims, lab feeds, and EMR data so the gap list reflects reality is step one — otherwise your team wastes time chasing gaps that are already closed, and misses ones that are truly open.
Separate "care gaps" from "documentation gaps"
